Our mission is to try and reduce the health disparities that cause chronic conditions, but we acknowledge that there is still a huge amount of work to do in supporting people who are suffering from existing chronic diseases.
There are many voluntary organisations dedicated to supporting specific chronic conditions, who we would welcome hearing from.
We are interested in supporting voluntary organisations that offer support services to individuals suffering from chronic disease and their families. This is the type of work will consider supporting:

The National Kidney Federation are the largest kidney patient charity in the UK. With approximately 3.25 million people living in the UK with kidney disease, the National Kidney Federation are a lifeline of support and hope to patients and families.
Hosted by The Forest of Avon our CEO, Delva Patman took part in a woodland wellbeing sharing day with heart patients and their families, in Bristol.
A £5,000 grant has been awarded to NKF and every other charity that took part received a £500 donation.
Thanks to players of The Health Lottery, we are giving away a grant of £5,000 to one charity working to support those impacted by chronic illness.
On Monday 30 June our CEO Delva Patman visited The Brain Charity. One of the 12 charities we awarded funding to in our first closed round.
Dying Matters Awareness Week (5 May – 11 May) highlighted the stigma about death, dying and grief – something that impacts every one of us. Rest of life care, delivered by hospices will be at the front and centre of conversations during this week.
